Advance Palliative Care (Home Based and Rehabilitation Services)

Pain management and control being physical, psychological, and spiritual stand to be the priority. We offer bereavement services for individuals and their families with multiple losses. Advanced Care Plan [ACP] is a type of care offered to individuals with life threatening illness. It is also offered to any person who needs to make choice on the way he/she would want to receive medical care or settle their social issues if it happened that they cannot do so due to illness or sudden death.

We also offer home-based health services for vulnerable patients with life threatening health problems. We make the patients home a safe and comfortable place to recover from pain or medical treatment. We achieve this through providing technical support and tools to empower families to provide optimal care for their ill family member. We regard end of life as a normal life course, thus we support families to journey with their loved one during his/her end of life without suffering from preventable pain. Homes can be the most conducive places for individuals and their loved ones during end of life if are prepared in advance. Our staff is available 24/7 and all of our visits are scheduled based on the clients’ needs.
